The Robert J. Dole VA Medical Center (VAMC) is currently recruiting for a Social Worker - Patient Aligned Care Team (PACT) to support our Community Based Outpatient Clinic (CBOC) in Hutchinson, KS.
Duties The Social Worker - PACT provides services in a wide variety of assigned settings including inpatient, outpatient, the Patient Aligned Care Team (PACT), Community Based Outpatient Clinics (CBOC) and other settings. The incumbent performs comprehensive psychosocial assessments, discharge planning, counseling with individuals/families/groups, provides education to Veterans/families, coordinates and arranges community based services, facilitates VA non-institutional care services, participates in multidisciplinary teams, provides advocacy for Veterans, provides consultation and education to staff regarding psychosocial aspects, represents the mission of the VA in all interactions.
