Work Shift

8:30 am–5:00 pm. No on‑call. Infrequent participation in early evening or weekend programs; up to 2 times per month (subject to dept needs with prior notice); will be discussed further during interview process.

This position will be remote after 90‑day review.

Required Qualifications

Graduate of an institution certified by the Council of Social Work Education with a Master’s degree in social work.

Licenses and Certifications: LMSW, licensed by the Texas State Board of Social Work Examiners. Temporary license acceptable during first 90 days of employment.

BLS (Basic Life Support) – required (not required in Transplant Departments).

Preferred Qualifications

2 years post‑graduate experience in a medical setting (inpatient, outpatient, home health care, hospice, etc.).

Oncology experience.

Additional certifications: LCSW, OSW‑C, CCM, etc.

Job Duties

Participate in multidisciplinary rounds and proactively determine patient needs for discharge transitions (transportation, DME, LTAC, SNF, home health agencies and hospice).

Utilize crisis intervention and problem‑solving skills to facilitate and enhance patient and family ability to meet acute and ongoing medical and psychosocial needs.

Maintain familiarity with community resources to facilitate positive patient and family outcomes and assure appropriate client access.

Maintain effective documentation, tracking and referral information; consistently meet department and UTSW documentation standards.

Identify and report situations or cases to appropriate governmental agencies as legally required and in compliance with organizational policies and procedures.

Serve as an active participant of the health care team, communicating effectively with other health care providers, patients, families, other hospital personnel, and visitors.

Identify need for professional growth and seek appropriate development opportunities.

Provide education to staff, patients and community resource affiliates.

Maintain system for tracking placement of patients after discharge on a monthly basis.

Have knowledge of Department and Unit Performance Improvement program and be aware of expected participation.

Perform other duties as assigned.
