(1547) Case Manager, Youth and Family Wraparound Facilitator

Overview
As a valued member of the Bluebonnet Trails Community Services Youth and Family services team, this professional position conducts a comprehensive assessment of the youth and their families, working in full partnership with team members to develop a plan of care, overseeing implementation of the plan, identifying providers of services or family based resources, facilitating child and family team meetings, and monitoring all services authorized for a child’s care.
Responsibilities The Wraparound Facilitator assures that care is delivered in a manner consistent with strength-based, family driven, youth guided, and culturally competent values, offers consultation and education to all providers regarding the values and principles of the model, monitors progress toward treatment goals, and ensures that necessary data for evaluation is gathered and recorded.
Qualifications Minimum Qualifications: A Bachelor’s degree from an accredited college or university with a major in a social, behavioral, or human service field or documented coursework equivalent to a major in human services. Knowledge of the effects of mental illness. Must have a valid Texas Driver’s License and maintain approved drivers status throughout employment.
Preferred Qualifications: One year of experience in mental health assessment and/or crisis intervention and screening; Master’s degree in psychology, social work or a related human service field. Bilingual preferred.

Hourly Rate $22.22 – $27.97/hour

$1.00/hr 6 years at BTCS
$1.00/hr – Advanced Licensure
