Overview
The Department of Developmental Services (DDS) offers the Human Services Coordinator I (HSC I) position located in the Merrimack Valley Area Office in Lawrence, MA. The HSC I serves as a primary contact for individuals and families, building respectful partnerships and coordinating services through the DDS funded residential, day and support program providers. Responsibilities
Coordinate and facilitate the development of Individual Service Plans (ISP) for assigned caseload. Monitor the implementation of services provided by DDS funded residential, day and support program providers. Arrange and provide crisis intervention as needed, assessing needs and making recommendations for follow‑up planning. Maintain regular and consistent contact with individuals, service providers and families. Advocate in human, civil and legal rights and maintain records in relevant computer databases. Represent the Area Office by building collaborative partnerships with individuals, families, guardians, provider agencies and other stakeholder groups. Qualifications
At least three (3) years of full‑time or equivalent part‑time professional experience in human services work or social work. Either a bachelor’s degree or higher in a related field may substitute for two (2) years of required experience. Current and valid motor vehicle driver’s license at a class level specific to assignment. Knowledge of Chapter 688 (Turning 22) law and transition processes for young adults with disabilities.
