Harbor Health Services is a mission‑driven organization that works with communities to promote health and well‑being. We are looking for an experienced Geriatric Social Worker to join our Elder Service Plan (a PACE program) in Mattapan, MA.
Location Elder Service Program, 1135 Morton Street, Mattapan, MA

Responsibilities
Participate in intake screening and conduct bio‑psychosocial assessments in participants’ residences or homes.
Perform timely initial, annual, and semi‑annual psychosocial assessments, using tools such as PHQ‑9, MOCA, and CDR.
Actively engage in required meetings, including Morning/Weekly Team, Plan of Care, and Family Meetings.
Attend monthly service plan meetings for participants in assisted living and nursing home facilities.
Provide individual and family supportive counseling at ESP and clients’ residences and conduct participant and caregiver support groups.
Manage clients during psychiatric hospitalizations.
Advocate for interdisciplinary team, clients, and families with community resources.
Collaborate with financial staff for bi‑annual financial redeterminations.
Travel between Harbor sites and participants’ residences and other off‑site locations as needed.
Requirements
MSW from an accredited school of social work, LCSW or LICSW required; current BLS certification required.
At least 1 year of experience working with frail elder populations.
At least 2 years of professional experience, including 1 year providing direct behavioral health therapy to adults.
Basic proficiency in Microsoft Office applications: Word, Excel, Outlook.
Preferred: 2+ years of geriatric experience.
Preferred: Experience with dementia care, substance abuse, and end‑of‑life care.
Critical thinking and problem‑solving skills for individual and team settings.
Bilingual a plus (English/Spanish or English/Haitian Creole).

Employment Type: Full Time | Years Experience: 1–3 years | Salary: $70,300 – $105,500 annually.
