Geriatric Social Worker at Harbor Health Services, Inc.

Pay Range

Base pay: $70,300.00/yr - $105,500.00/yr.

The above posted pay range information is for LICSW licensure with a minimum starting salary of $80,000. The pay range for LCSW licensure is $62,500 - $93,800 with a minimum starting salary of $64,480.

Responsibilities

  • Participate in intake screening process at potential participants' residences.
  • Conduct bio-psychosocial assessment.
  • Perform timely initial, annual, and semiannual psychosocial assessments in client residences, using tools such as PHQ-9, MOCA, and CDR.
  • Actively participate in required meetings including Morning/Weekly Team, Plan of Care, and Family Meetings.
  • Attend monthly service plan meetings for participants in assisted living and nursing home facilities.
  • Provide individual and family supportive counseling at ESP and client residences; conduct participant and caregiver support groups.
  • Manage clients during psychiatric hospitalizations.
  • Advocate for interdisciplinary team, clients, and families with community resources.
  • Work with financial staff to assist with bi-annual financial redeterminations.
  • Travel between Harbor sites and participant residences as needed.

Requirements

  • MSW from an accredited school of social work.
  • Current MA LCSW or LICSW license required; current BLS certification required.
  • MoCA certification required or to be obtained within 3 months of hire.
  • Two years of professional experience, with at least one year providing direct behavioral health therapy to an adult population.
  • One year of experience working with the frail elder population (paid or volunteer); two or more years of geriatric experience preferred.
  • Basic skills in Microsoft Office 365 (Word, Excel, Outlook, Teams).
  • Experience with dementia care, substance abuse, and end-of-life care preferred.
  • Critical thinking skills to problem-solve, both individually and in a team setting.
  • Bilingual a plus (English/Spanish, Haitian Creole, or Portuguese).

Employment Details

Monday - Friday, Full Time – 40 hours per week.
