Elder Service Plan – Mattapan, Mattapan, MA, US

Full Time – 40 Hours Weekly (Monday – Friday)

Salary Range: $70,300 – $105,500 annually (LICSW $80,000 minimum, LCSW $64,480 minimum). $2,500 sign‑on bonus offered.

Role

The Social Worker plans, coordinates, and manages social work services for frail older adults and caregivers within the Elder Service Plan (ESP) as part of the Integrated Disciplinary Team (IDT). Responsibilities include conducting initial, semi, and annual assessments; participating in the intake process for potential participants; providing supportive counseling to ESP participants and their families; and actively participating in interdisciplinary Plan of Care meetings and other team meetings.

Responsibilities

  • Participate in the intake screening process in potential participants' residences or homes and conduct a bio‑psychosocial assessment.
  • Conduct timely initial, annual, and semiannual psychosocial assessments at client residences to develop participants' plan of care using tools such as PHQ‑9, MOCA, and CDR.
  • Actively participate in required meetings, including Morning/Weekly Team, Plan of Care, and Family Meetings.
  • Attend monthly service plan meetings for participants in assisted living and nursing home facilities.
  • Provide individual and family supportive counseling at ESP and client residences and conduct participant and caregiver support groups.
  • Manage clients during psychiatric hospitalizations.
  • Assist/advocate for the interdisciplinary team, clients, and families with community resources.
  • Work with financial staff to assist with bi‑annual financial redeterminations.
  • Travel between Harbor sites and participants' residences as needed in a timely manner.

Requirements

  • MSW from an accredited school of social work.
  • Current MA LCSW or LICSW license required; current BLS certification required.
  • MoCA certification required or obtained within 3 months of hire.
  • 2 years of professional experience, including 1 year in direct behavioral health therapy to an adult population.
  • 1 year of experience working with the frail elder population (paid or volunteer); 2+ years of geriatric experience preferred.
  • Basic skills for Microsoft Office 365 (Word, Excel, Outlook, Teams).
  • Experience with dementia care, substance abuse, and end‑of‑life care preferred.
  • Critical‑thinking skills to problem‑solve individually and in a team setting.
  • Bilingual a plus (English/Spanish, Haitian Creole, and/or Portuguese).
