Location: Bingham Farms, MI. Salary: $63,000.00-$73,000.00.

Job Description Under the supervision of the Intake Manager, the PACE Southeast Michigan (PACE SEMI) SW Intake Coordinator is responsible for completion of the PACE SEMI intake process, which includes initial home visits and assessments, assistance in obtaining Medicaid documents, and communication with the Intake team. The coordinator communicates information about the PACE program and other community services to potential participants through home visits and phone contact and assesses eligibility for enrollment into the PACE program.

Specific Duties & Responsibilities

Meet with prospective enrollees and utilize social work assessment skills to determine eligibility for the PACE program.

Manage all aspects of the Home Visit process: explain PACE SEMI, complete all initial intake assessments, and sign enrollment paperwork.

Prepare written communication to the Intake team introducing each intake candidate and update daily on Home Visit results.

Input potential participant’s information and assessments into the electronic medical record (EMR).

Communicate regularly with Participant Advocate to ensure the Interdisciplinary Team (IDT) has all needed information on the potential participant.

Work closely with the eligibility specialist to ensure Medicaid and Medicare eligibility upon intake.

Coordinate timely follow-up with potential participants and their families through phone contact when needed.

Maintain confidentiality of all PACE SEMI participant information and related business practices.

Perform other duties as assigned.

Knowledge, Skills, and Abilities

Master’s degree in Social Work.

Limited or full social work license required.

One or more year(s) of experience working with elderly in a community-based setting.

Knowledge of senior community and area resources.

Basic knowledge of Microsoft Windows and Microsoft Outlook.

Self-directed, effective communicator, flexible, team-oriented, initiative, and willingness to learn.

Ability to market the program to improve enrollment rates.

Driving required within Wayne, Oakland, and Macomb counties.
