Families & Youth Innovations Plus (FYI+)
Full‑time position – hourly rate $17.98–$21.76 (three levels based on experience and education). FLSA status: Hourly/Non‑Exempt. Reports to LVP Program Supervisor. Department: La Vida Project – El Puente Program.
Job Summary:
The Success Coach provides specialized case‑management services to meet individual client needs, creates programs to benefit clients, and educates clients on available community resources. Success coaching ensures that clients receive LVP services in a timely, efficient, and coordinated fashion.
What You’ll Do
Provide clients with supportive services.
Coordinate services between providers and clients.
Interpret agency services to clients and community.
Provide case‑management and skill‑building services to youth in the program.
Provide housing case‑management services.
Maintain individual case records for clients receiving services.
Provide specialized case‑management services to meet individual client needs.
Educate clients on available community resources and services.
Create programs and services to benefit clients and the community.
Attend community meetings relevant to the program and clients.
Administer and facilitate completion of appropriate intake forms, assessments, intake packets, and service plans.
Conduct field visits, attend meetings, and be available to deliver services during traditional and non‑traditional hours in the client’s home, educational institutions, places of employment, FYI+ main office, JPPO offices, detention facilities, reintegration centers, or any other setting that facilitates consistent, structured delivery of LVP services.
Organize and facilitate Life Skills groups as needed, both individually and in groups.
Participate and interact in youth activities (e.g., camping, sport activities, overnight youth conferences) which may involve occasional overnight activities and trips.
What You Need
Bachelor’s degree in Social Work, Family and Consumer Sciences, Criminal Justice, Psychology, Sociology, or Criminology from an accredited college or university.
Pass a CYFD criminal records check.
Maintain full certifications in CPR/First Aid and CPI.
Maintain a valid NM driver’s license.
Location:
Las Cruces, NM. Salary range $43,000–$52,030.
