Position Summary
Eventus WholeHealth delivers an integrated model that provides holistic care for medically vulnerable adults achieving extraordinary outcomes for the people we serve. This care is provided to adults who reside in skilled nursing, assisted living facilities and private residences through a network of healthcare providers including physicians, nurse practitioners, physician assistants, psychologists, licensed clinical social workers, and clinical and administrative support staff.
Responsibilities
Support and facilitate clinical services with assigned providers, working in‑field up to 60 miles from home.
Ensure the clinical provider in the assigned building can focus on patient care for the day.
Confirm each patient is seen as medically necessary per clinical protocol.
Act as a buffer between provider and families/staff to facilitate proper patient management.
Document appropriate encounter and time for CCM and BHI services.
Maintain and update scheduling rosters daily, ensuring accuracy and timely distribution.
Assist with day‑of changes to scheduled visits due to acute needs, hospitalizations, admissions or discharges.
Prepare clinical documentation each day and complete basic opening of encounter notes per discipline protocol.
Perform medication reconciliation on all scheduled patients each encounter.
Enter facility chart data, lab and other diagnostic results into patient encounter notes and assign to provider as guidelines dictate.
Assist with collection and sorting of all clinical paperwork needed for clinician review and ensure completion each day.
Host telehealth sessions for providers as assigned by manager/supervisor, both routine and acute visits.
When scheduling is in scope, share patient schedules with key facility staff as needed.
Assist with triage needs and documentation as directed by the assigned provider.
Ensure all necessary paperwork is routed to Eventus providers, office or families.
Assist with consents for any services needed.
Attend stand‑up meetings to report needed information between facility staff and Eventus providers.
Assist with discharge planning of clinical services as needed.
Ensure daily check‑in and check‑out with facility staff regarding patients seen and status.
Maintain patient confidentiality at all times; communicate transitions of care to the CCM team.
Check all forms of communication several times daily to ensure patient needs are met.
Inform patient and family of recent visits, new orders and defer to provider when needed.
Identify and articulate responsibilities, adhering to Eventus WholeHealth standards, policies and procedures including HIPAA rules.
Adhere to the dress code policy—clean, wrinkle‑free scrubs, appropriate closed‑toed footwear and name badge.
Report early completion of duties to Lead Clinician Support Specialist for additional assignments.
Seek further training if uncertain how to proceed with any assignment.
Qualifications / Education
High School Diploma or equivalency.
Valid driver’s license and reliable transportation.
Expected travel up to 1 hour from home address.
Typing speed of 45 wpm or greater.
Certified Clinician Support Specialist degree is a plus.
Medical Scribing Experience Preferred.
