About The Assertive Community Treatment (ACT) Program Assertive Community Treatment (ACT) is a multidisciplinary team of medical, behavioral health, and rehabilitative professionals who work collaboratively to meet the complex needs of individuals with severe and persistent mental illness—predominantly schizophrenia—and high service needs to maintain stability in the community.

ACT provides a wide range of services, including service coordination, psychiatric assessment and treatment, medication management, substance use services, counseling and therapy, vocational services, mental health support services, peer support, and representative payee services. The program’s flexible model ensures that individuals receive the right level of care based on their changing needs over time.

The primary goal of ACT is to lessen or eliminate the debilitating symptoms of mental illness, minimize recurrent episodes, and help individuals meet their basic needs, improve social and vocational functioning, increase stability in the community, and reduce hospitalizations—ultimately enhancing quality of life for both clients and their families.

About The Role As an Employment Counselor, you will play a vital role on the ACT team by assessing clients’ vocational interests and skills, connecting them to meaningful employment opportunities, and guiding them through every step of their vocational journey. In this role, you will assess client needs and develop individualized vocational placement and training plans while providing supportive counseling, job coaching, and on‑the‑job supervision.

You will deliver rehabilitative and habilitative vocational training to clients with mental health and co‑occurring disorders, conduct individual and group sessions to enhance job readiness and independence, and navigate governmental regulations related to the employment of individuals with disabilities to ensure equitable access to resources. Additionally, you will maintain accurate and timely client records and documentation. Your work will directly contribute to improving client functioning, increasing community stability, and supporting recovery through meaningful employment.

Qualifications

Bachelor's degree in Human Services (such as Social Work, Psychology, or Counseling)

One (1) year of experience in case management and vocational training with adults with mental health and co‑occurring disorders

Must be registered with the Board of Counseling as a Qualified Mental Health Professional (QMHP) or eligible to register as a QMHP within the first 30 days of employment

Valid state Driver’s License required

The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test, and employment reference checks.
