Case Manager – Supportive Housing (Women In Need, Inc. (Win))

Location: New York, NY

Pay Range $25.98/hr — $25.98/hr (actual pay based on skills and experience)

Job Overview The Case Manager provides case management services for clients in transitional housing, integrating housing and employment goals while ensuring compliance with Win’s contractual requirements and funder obligations.

Responsibilities

Conduct initial assessment within 48 hours of arrival; complete Family Case Assessment, Initial Service Plan, and intake documentation.

Initiate referrals for emergency services and complete intake information in the Uniform Client Chart.

Print and file CARES data and supporting documents.

Orient clients to facility rules, rights, grievance process, emergency procedures.

Assist clients with transition to residential units and housekeeping.

Provide case management, crisis intervention, counseling and support.

Use evidence‑based practices such as strength‑based case management, motivational interviewing, trauma‑informed care, critical time intervention, family‑centered case management, boundary setting, and cultural‑reflexivity.

Develop an Independent Living Plan (ILP) for each client and track progress.

Provide training, coaching and skill‑building opportunities to support ILP goals.

Ensure clients meet responsibilities outlined in the Client Acknowledgment Responsibility Form.

Maintain client data in the Uniform Client Chart and update CARES entries accurately.

Assist families with income and public benefits (e.g., SSI).

Refer clients to medical, mental health, education/job training, legal, advocacy services and other community resources.

Track referrals and follow up on client progress.

Assess job readiness using standardized assessment tools.

Enter client information to meet OTDA and DHS regulatory requirements; collaborate with Housing Coordinators on permanent housing pursuit.

Attend shelter, agency and staff training meetings.

Work evenings, one Saturday each month, and holidays as scheduled.

Prepare mandated reports for supervisor, contract agency and funding agency.

Conduct client unit inspections; review findings with client and supervisor.

Perform other related duties as needed.

Essential Functions

Ascend/descend stairs in assigned building.

Escort clients to appointments, including apartment viewings.

Qualifications

Commitment to Win’s mission, vision and values.

Bachelor’s degree in social work or related field (preferred) with 2 years relevant experience, or Associate’s degree with 3 years client‑facing experience.

Familiarity with evidence‑based practices (strength‑based case management, motivational interviewing, trauma‑informed care, critical time intervention, family‑centered case management, boundary setting, cultural‑reflexivity).

Experience with entitlement systems and procedures.

Experience working with families and domestic violence and homelessness issues.

Experience with standardized assessment tools.

Excellent organization, written and verbal communication.

Team‑work effectiveness.

Computer skills and knowledge of CARES a plus.

English/Spanish bilingual proficiency a plus.
