Case Manager
in Salt Lake City, Utah!!

The housing Case Management Lead assists the case management supervisor with training housing case managers and managing other daily aspects of the housing case management team, including but not limited to monthly reporting, supervision of housing case managers, managing caseloads and referrals for housing programs or vouchers, and managing housing applications that have been submitted. The housing case management lead maintains other housing case management job duties and a reduced case load to be able to manage the extra job responsibilities.

Work Hours & Compensation
Pay Rate: $60,000 - $62,000 per year
Monday - Friday flexible hours
36 hour standardized work week

Education and Experience
Case management experience is required
A minimum of a bachelor's degree in social work or related field is preferred
Experience in the substance-abuse field is preferred
Duties
Manage subcontracts for housing grants and programs
Assist in training and onboarding of housing case managers
Supervision and development of housing case management staff
Monthly case staffing with housing case managers to discuss caseloads and housing barriers
Monthly reporting and coordination of case loads
Review staff productivity, billing and services monthly
Frequent communication with partner agencies to coordinate housing for clients
Attend weekly triage/coordinated entry as needed
Work with data team to track and monitor HMIS data for grant reporting
Mediate conflicts between clients and case managers
Problem solve housing issues between clients and landlords
Coordinate UTA bus passes for housing clients
Manage housing moves, storage units and requests for bed and furniture vouchers
Complete comprehensive needs assessment to identify client needs, strengths and barriers
Develop an individualized case plan with clients to help ensure housing stability
Collaborates with outside agencies to connect clients with employment services, education resources, healthcare programs, housing programs, and other relevant community resources
Collaborates with subcontractors, counselors, case workers, and outside agencies to deliver case management and coordination of services
Provide ongoing case management and service coordination, including regular home visits
Provide client advocacy when needed
