About Our Team Our care model meets members wherever they are—whether at home or in the community. As part of our team, you will support engagement outreach, schedule intake appointments, aid in form completion, and keep community partners involved.

Role Description The Engagement Specialist connects with members to ensure they receive the care and services they need. This role focuses on outreach, scheduling, documentation, attending community events, and maintaining partner relationships. The ideal candidate is bilingual (English/Spanish), detail‑oriented, and passionate about making a difference.

Responsibilities

Outreach and Engagement

— conduct in‑person outreach for members on eligibility lists, complete Athena documentation, schedule intake appointments, attend outreach events, and build relationships with community coalitions and stakeholders.

Documentation and Coordination

— convert leads to patients using internal systems, document all outreach attempts and outcomes in Athena, and assist in sending mailers and follow‑up calls from community events.

Community Presence

— represent Zócalo Health at coalition meetings and collaborative events, ensuring professional messaging and building trust with stakeholders.

Requirements

Minimum of 1–3 years experience in healthcare, community outreach, or a related field.

Experience engaging diverse communities with culturally sensitive communication.

Skills and Competencies

Fluent in English and Spanish (verbal and written).

Strong interpersonal and communication skills to build trust with patients and partners.

Detail‑oriented with excellent organizational skills.

Comfortable with technology; proficient in electronic health records (Athena experience preferred).

Additional Requirements

Ability to work a flexible schedule, including evenings and weekends for events.

Access to reliable transportation for in‑person outreach and events.
