Case Manager (Part Time – 25 hours) Huntington, WV – Regular, Part‑Time, Non‑Exempt

$20.00/hour – Open Until Filled

Qualifications

Ability to work well with others in a concerted team effort.

Knowledge and skills using community resources available to assist clients in case management.

Good written and oral communication skills and self‑motivated.

Ability to work well on own with minimal supervision.

Must have current valid driver’s license with good standing driving record.

Must pass drug screen and background check.

Ability to adapt to the program’s changing needs.

Ability to operate all programs and databases required.

Must be proficient with MicroSoft Office Suite.

Knowledge of Medicaid Waiver Regulations and Policies.

Position Details (This is a sampling of duties and responsibilities of this position)

Coordinate health care & social services for clients from entry into the system until the case is closed.

Complete a comprehensive person centered assessment of all appropriate referrals.

Develop a Service Coordination Plan (SCP) by negotiation with the client and Homemaker and Adult Protective Services (APS) healthcare assessment.

Meet with client and Homemaker Agency RN for 6 month and annual reviews and to sign SCP.

Conduct face to face visits/reviews with clients every 6 months.

Conduct case reviews to ensure policy and procedures are being followed.

Complete monthly contact with clients by phone. Those not accessible by phone require a home visit.

Oversee the implementation of SCP by maintaining contact with all providers required.

Monitor service provisions to insure that services are provided as planned.

Process and record incoming referrals and follow up with the potential clients to include: Initial home visit, Monthly phone calls to referral, Maintain current chart notes on referrals, Conduct quarterly client visits.

Responsible for Case Management report that goes to Bureaus of Senior Services (BOSS) by the 5th of each month – includes new clients, transfers, deaths and closed cases.

Represent SCAC in a positive manner in all aspects of job related activities.
