The TwelveStone Infusion Centers LLC manages the infusion needs of patients and maintains the continuum of care, from intake, assessment, and treatment through the completion of their visit. Our primary focus is the safe and effective administration of biologic medications (e.g., Remicade, Tysabri, etc.), but we also offer a full range of infusion services including antibiotics, hydration, vitamins, iron, and steroids. On occasion there is a need for sterile dressing changes and lab draws.
Responsibilities
Perform analysis of patient data including status of lab work, patient assessments
Report changes to referring physician
Excellent IV Skills
Perform initial and ongoing patient assessments
Provide patient education; participate in clinic quality improvement process.
Conduct individualized patient assessments, prioritizing the data collection based on the patient’s immediate condition or needs within the timeframe specified by client facility’s policies, procedures or protocols.
Administer medications, infusion therapy, and other appropriate treatments per prescriber orders, and under the supervision of the clinic mid-level provider
Document, verify and ensure implementation of physician orders, accurate and timely clinical documentation. Document patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution.
Deliver care with appropriate age and cultural competence to specific patient populations according to individualized needs. Maintain confidentiality in matters related to patient, family and client facility staff. Provide care in a non‑judgmental, non‑discriminatory manner that is sensitive.
Qualifications
Bachelor’s degree in nursing, Associate degree in Nursing and current, unrestricted RN license in the state of practice.
Must have a current, CPR certification (BLS minimum, including hands‑on/in‑person assessment).
Minimum of one year’s experience providing nursing care preferred. Infusion experience preferred; telephone, MS Office and computer/keyboard skills required. Previous EMR experience preferred.
Effective verbal, written, and electronic communication skills.
Outstanding organizational skills with the ability to multi‑task in a fast‑paced, high‑energy environment.
Initiative, creativity, problem‑solving ability, adaptability, and flexibility.
Ability to work with and without direct supervision and practice autonomously.
Ability to collaborate with professional colleagues as necessary to provide quality care.
Ability to lift packages of up to 10 lbs.
Prior experience with typing, computers and electronic medical records.
Ability to consistently present a friendly, welcoming, and professional approach to patient care.
