Job Description

Provide skilled nursing care in accordance with established standards and facility policies.

Administer medications and treatments as prescribed, ensuring timely and accurate documentation.

Monitor patient conditions, including vital signs, reporting significant changes to supervising RN or provider.

Assist with patient assessments and contribute to care planning.

Perform wound care, dressing changes, and other clinical procedures as required.

Support residents with ADLs and ensure a safe, comfortable environment.

Maintain accurate and organized medical records within regulatory guidelines.

Respond effectively to emergencies and follow established protocols.

Collaborate with interdisciplinary staff to ensure continuity of patient care.

Provide compassionate and professional support to patients and their families.
