Compensation:
$30.00/hr - $51.10/hr, based on skills and experience.
Responsibilities
Identify patients with complex psycho/social needs and facilitate eligibility determination for social programs.
Serve as the mental health professional of the interdisciplinary team, providing crisis intervention and coordination of care plans.
Conduct psychosocial assessments and deliver interventions via in‑person, phone, or telehealth.
Contribute to interprofessional team discussions, discharge planning, and family meetings.
Advocate for patient and family empowerment, address social, emotional, and financial consequences of illness.
Assist with chaplain planning, pain and symptom management, and bereavement support.
Participate in on‑call rotation as required.
Qualifications
Master’s degree in Social Work (MSW) required.
Current New Mexico license: LCSW, LICSW, or LMSW.
10 years of hospital social work or discharge planning experience in relevant setting (for Social Worker III level); 5 years for II; 1 year for I.
Experience in case management and national certifications (ACHP‑SW, APHSW‑C) preferred.
Familiarity with hospice, home‑health, and palliative care settings.
