Job Summary This role involves the development, planning, implementation, and evaluation of psycho/social interventions for patients in Home Health, Hospice IP/OP, and Palliative programs. It requires identifying patients with complex psycho/social needs, coordinating eligibility for social programs, and serving as the team’s mental health professional.
Key responsibilities include crisis intervention, coordination of patient plans of care, and facilitation of family meetings. The position may involve in‑person, phone, or telehealth services.

Additional Job Duties
Provide Home Health, Hospice IP/OP, and Palliative Care social work across outpatient and inpatient settings.
Assess patients and families for psychosocial risk factors and complete comprehensive psychosocial assessment.
Provide psychosocial interventions and psychoeducation.
Offer supportive counseling for illness adjustment, catastrophic diagnosis, changes in living situations, and bereavement.
Intervene with patients with complicated grief and explore bereavement related to possible death and multiple aspects of loss and adaptation.
Provide anticipatory guidance and non‑pharmacological pain and symptom management.
Assist Hospice Chaplain with developing plan of care.
Advocate for patient and family empowerment and independence and access needed services within the health care system.
Facilitate family meetings and participate in discharge planning activities.
Work with families exhibiting complex dynamics impacting patient care and discharge.
Provide clinical guidance and consultation to interdisciplinary primary teams.
Educate patient/family and interdisciplinary team regarding post‑acute options and address issues of choice.
Provide intervention in child abuse/neglect, domestic violence, guardianship, foster care, adoption, mental health placement, advance directives, adult/elderly abuse, child protection and sexual assault as appropriate.
Participate in on‑call rotation as required by department.
Supervise and mentor Social Workers enrolled in Master level programs to fulfill practicum clinical hours for licensure and diploma requirements.
Qualifications
Master’s degree in Social Work required.
Current license from New Mexico as a Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW) or Licensed Master Social Worker (LMSW).
Hospital social work or discharge planning experience in a relevant setting required.
Hospital social work experience, protective services, or practicum in acute care settings desired.
National Case Management certification or SW Palliative Care Certification; ACHP‑SW or APHSW‑C preferred.
