for Full‑time externals, pro‑rated for Part‑time. Full Time Medical Practice LPN position in the Float Team department. 40 hours per week, Monday–Friday 8‑hour shifts between the hours of 8:00 am–5:00 pm. NY Areas of Coverage: Poughkeepsie, Highland, Hyde Park, Rhinebeck, Kingston, Woodstock, Modena, Millerton, Millbrook (NY). CT Area of Coverage: Sharon, CT. To assist the clinician in providing medical care to the patients of the Medical Practice. Responsibilities

Prepares materials, supplies, equipment, and the patient to assist clinician with the patient's medical exam. Obtains the patient's history and medical information. Provides direct patient care as directed by a Registered Nurse or clinician. Collects and processes specimens as directed by the clinician and performs point‑of‑care testing. Administers medications and vaccinations as directed by the clinician. Instructs the patient and family on how to perform clinician‑recommended preventive care, takes prescribed medication, prepares for tests, monitors treatment progress and notes symptom changes. Contacts the patient to provide requested follow‑up, advises them of test results or relays clinicians instructions. Fulfills and adheres to all compliance responsibilities related to the position. Maintains and models the Organization's values. Demonstrates regular, dependable, and predictable attendance. Performs other duties as required. Required

Basic Life Support (BLS) certification within 30 days of starting. Licensed Practical Nurse licensure. Basic medical knowledge and terminology. Computer skills/data entry. Skilled in obtaining accurate patient vitals. Vaccine administration and medication preparation. Preferred

Minimum Experience: one year. Proficiency in multiple languages. Phlebotomy. EKG. Education Required

HS Graduate or Equivalent. Graduate of an accredited LPN Program. Other Information

Physical Effort: Medium to Heavy effort. May exert up to 35 lbs force. Salary Range: $26.48 - $50.49 hourly + $4 Float Differential.
