Overview Continuum Rehab Group provides in‑house therapy with Skilled Nursing facilities in Texas, New Mexico, Arizona, Indiana, and Missouri.
Compensation Base pay: $52.00 hr – $58.00 hr. Total compensation includes 401(k), PTO, medical, dental, and vision benefits.

Experience / Qualifications
Graduated from an accredited university.
Valid license in the appropriate state (or in process).
Open to therapists at all experience levels.
Responsibilities
Utilizes evidence‑based practice to support clinical interventions.
Provides rehabilitative, skilled, and medically necessary treatment to patients with physical functioning disorders per regulatory and clinical practice requirements.
Delivers skilled therapy services to patients requiring rehabilitation.
Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
Supervises Physical Therapy Assistants, temporary licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
