Assessment Clinician – Olive Ave Health Center (Glendale, AZ)

Reporting to the Clinical Site Manager, the ideal individual is flexible, compassionate and professional. Location 6153 W Olive Ave, Glendale, AZ 85302.
Qualifications
Arizona license: LCSW, LPC, LISAC, LMSW, LAC, LASAC or LMFT in good standing.
Master’s degree (MA, MC, MSW) in a behavioral health field.
Experience in treating co‑occurring disorders preferred; behavioral health experience preferred; new graduates considered.
Licensed therapists eligible for weekly clinical supervision.
Responsibilities
Review DCS referral and supplemental documents prior to assessment.
Conduct a comprehensive biopsychosocial and ASAM Criteria® assessment to determine treatment needs.
Request additional information from the referring DCS/DES Specialist if needed.
Coordinate referrals to appropriate stabilization or inpatient facilities.
Prepare a written assessment summary following the ASAM Criteria® Comprehensive Assessment Report outline.
Complete care coordination activities with other entities as needed to ensure client needs are addressed.
