HOPE ~ HEALTH ~ HEALING

Location: Glendale, AZ 85302
Schedule: Full-Time, 40 hours/week – 7:30am - 4:00pm M-F
Experience Treating Co-Occurring Disorders Preferred
Behavioral Health Experience Preferred, Licensure Preferred
(Master’s degree, i.e., MA, MC, MSW, in a Behavioral Health Field)
Arizona Licensed: LCSW, LPC, LISAC, LMSW, LAC, LASAC or LMFT in Good Standing (Temporary license does not qualify; New Hires Only)
Additional Language Differential Pay Available: Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili or ASL
Position Summary Behavioral health experience is defined as work experience providing direct care to individuals whose condition is related to mental disorder, a personality disorder, substance abuse, or a significant psychological or behavioral response to an identifiable stressor. Employee will serve as a clinician at Terros Health, supervised by Clinical Manager or Regional Administrator. This position is responsible for providing outpatient counseling services which may include adults, children, adolescents, families, couples, and groups using the Triple Aim framework of “Better health for the population, better care for the individual and lower cost per capita.” Ensures the successful achievement of Terros Health vision, mission, program objective and compliance with a broad range of current federal and state health regulations and policies. Provision of services will occur in the office or other place of service.
Clinician Duties
Responsible for providing outpatient counseling services to clients.
Responsible for intakes, assessments, treatment and discharge planning, and group counseling through evidence-based practices.
Charts progress, maintains clinical records and ensures implementation of treatment and discharge plans.
Monitors and follows up on client needs, and submits monthly service reports as required or needed.
Coordinates services as necessary working in collaboration with various State, Federal, and local agencies.
Provides client-centered outpatient clinical services to individuals/families.
