Arbor Post Acute is seeking a skilled and compassionate
PRN Physical Therapist
to join our interdisciplinary rehabilitation team. This role offers flexibility while allowing you to make a meaningful impact by helping patients regain mobility, independence, and quality of life.
What You'll Do
Evaluate patients promptly following physician referral and develop individualized treatment plans
Deliver high‑quality physical therapy services in accordance with physician orders and best practices
Document evaluations, daily treatments, progress notes, and discharge summaries in compliance with state and payer requirements
Collaborate with physicians, nursing, families, and the interdisciplinary care team to support patient progress
Supervise Physical Therapy Assistants, aides, and students in accordance with state practice acts
Participate in care conferences, utilization review meetings, and rehabilitation team discussions as needed
Provide education and training to patients, families, and facility staff to support continuity of care
Assist with discharge planning and caregiver training
Recommend and facilitate ordering of durable medical equipment to promote patient independence
Support staff development through in‑services and ongoing education
What We're Looking For
Bachelor's degree in Physical Therapy (Master's or Doctorate preferred)
Current, active Physical Therapist license in good standing
Experience in a post‑acute or skilled nursing setting preferred
Proficiency with electronic documentation systems such as
Casamba
and
PointClickCare
Strong communication, organizational, and teamwork skills
Ability to build positive relationships with patients, families, and colleagues
