is located at 6141 N. Pulaski Road, Chicago, IL. It offers a quiet, tree‑lined environment with easy access to public transportation, major thoroughfares, and local amenities such as retail shops, restaurants, universities, and museums. The center is accredited by The Joint Commission and holds a Gold Seal of Approval® for Post‑Acute Care. It specializes in post‑hospital rehabilitation, wound care, short‑term respite stays, and includes the Evergreen Unit dedicated to residents with dementia and Alzheimer’s disease.

Responsibilities
Perform Activities of Daily Living for residents.
Observe and report changes in residents’ conditions.
Maintain a safe and clean environment for residents.
Document care activities and maintain resident education records.

Excellent communication and teamwork skills.
