Description A Care Coordinator informs and educates patients about their condition, provides care and treatment plans, and oversees the process to ensure patients are connected with corresponding professionals.
Responsibilities
Participates in payer group meetings and performs gap closures by supporting patient billing and scheduling outreach.
Utilizes health databases to document notes, study accounts, and communicate with necessary parties to prevent and collect outstanding balances.
Monitors quality performance trends by maintaining reports up‑to‑date.
Assists patients with treatment plans by communicating with health professionals to provide referrals, coordinating home care and DME services, and aiding transition of care management.
Location Primary: Tampa, USF Health South Tampa Center 2, Tampa General Circle, Tampa 33606
Remote Work: Hybrid Remote
Schedule Full‑time, Monday – Friday
