Overview Maine Center for Integrated Rehab / Rehab Without Walls offers an outpatient clinic where we integrate therapy into patients’ home and community environments.

Who we are looking for:
Experienced LCSW passionate about providing counseling services that support individuals’ physical, mental, cognitive, and emotional adjustment through recovery.
Provides therapeutic interventions and resource recommendations addressing patient goals to achieve personal, career, and independent living goals in an integrated setting.
Values professional autonomy, teamwork, communication, and collaboration to maximize patient recovery.
Responsibilities (not limited to)
Performs comprehensive evaluations, assessing the social, emotional, and support needs of patients and their families.
Sets measurable objectives formulated in conjunction with the rehab team.
Assists physicians and other health team members in understanding significant social and emotional factors related to patient health problems.
Instructs / counsels patients and families in coping with social and emotional responses connected with illnesses.
Participates in care coordination activities and acts as a resource to other health team members in identifying and resolving patient needs.
Identifies and assists patients and/or families to utilize appropriate community resources to achieve measurable objectives.
Qualifications
Master’s Degree in Clinical Social Work (LCSW).
Current Licensed Clinical Social Worker Licensure in the state of practice.
Minimum of one year experience in a rehabilitation setting serving individuals who have experienced a traumatic brain injury, spinal cord injury, or acute diagnosis.
CPR certification required.
Ability to lift 50 pounds.
Ability to lift patients up to 100 pounds following safety procedures.
Requires fine motor skills, visual acuity, and the ability to walk/stand for extended periods.
Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.
