Overview Rehab Without Walls is hiring a LCSW (PRN/Part Time) to join our Home & Community Rehabilitation team in Fort Worth, TX.
We’re looking for a
Licensed Clinical Social Worker (LCSW) to provide specialized mental health counseling for social and emotional adjustment during rehabilitation from catastrophic injury or illness.
Support individuals and families as they adapt to disability and lifestyle changes.
Provide therapeutic interventions and resource support and navigation that address the patient’s goals.
Have knowledge of medical social work and/or neurological injury (TBI/CVA).
Service areas: Fort Worth, Burleson, Alvarado – work where you live.

Responsibilities
Perform comprehensive evaluations, assessing the social, emotional, and support needs of patients and families.
Set measurable objectives in conjunction with the rehab team.
Assist physicians and other health team members in understanding social and emotional factors related to patient health problems.
Instruct and counsel patients and families in coping with social and emotional responses to illness.
Participate in care coordination activities and act as a resource for other health team members in identifying and resolving patient needs.
Identify and assist patients and families in utilizing appropriate community resources to achieve measurable objectives.
Qualifications
Master’s Degree in Clinical Social Work (LCSW).
Current licensed clinical social worker license in the state of practice.
Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury or acute diagnosis.
Current BLS (Basic Life Support), First Aid, and CPR certification required.
Ability to lift 50 pounds.
Traveling, lifting, and transferring patients may involve lifting up to 100 pounds following safety procedures.
Requirements for fine motor skills, visual acuity, and walking/standing extended periods.
Additional physical requirements include pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently.
