Position Summary Family Tree Rehabilitation, part of The Campus of Anna Maria, is seeking a PRN Occupational Therapist Registered (OTR) to support our therapy team as needed. Located on a peaceful wooded campus in Ohio, we are a family‑owned and operated skilled nursing facility that takes pride in delivering high‑quality, compassionate, and individualized care.
The PRN Occupational Therapist Registered (OTR) will evaluate residents and develop, implement, and update individualized occupational therapy plans of care to help residents achieve maximum independence and quality of life.
Responsibilities include providing care in Skilled Nursing Facility (SNF), Outpatient, and Home Health Care settings.
Competitive starting salary up to
$50 per hour
based on experience.

Key Responsibilities
Perform occupational therapy evaluations and establish individualized plans of care
Provide skilled occupational therapy treatment in accordance with physician orders
Supervise and collaborate with COTAs as applicable
Accurately document evaluations, treatments, progress, and discharge plans
Participate in interdisciplinary care planning and team communication
Ensure compliance with facility policies, professional standards, and state and federal regulations

Qualifications
Current Ohio Occupational Therapist (OTR) license in good standing
Graduate of an accredited Occupational Therapy program
