Enhanced Care Management (ECM) Lead Care Manager - Shasta County
Pacific Health Group is an equal‑opportunity health organization focused on addressing social determinants of health. As a Lead Care Manager you will create comprehensive care plans and coordinate medical, social, and community services to meet each member’s needs.
Base pay range:
$25.00 - $29.00 per hour.
Expected hours:
40 per week.
Responsibilities
Conduct frequent in‑person visits to members’ homes, shelters, or community centers to assess needs and build trust.
Coordinate end‑to‑end care, scheduling appointments, arranging transportation, securing housing, and providing meal delivery or other essential services.
Advocate on behalf of members for timely treatments, insurance authorizations, and access to services, handling appeals and roadblocks.
Maintain detailed documentation and reporting to support transparency and accountability.
Communicate effectively with families, healthcare teams, and community organizations, ensuring seamless handoffs.
Use data and feedback to refine care strategies, participating in continuous improvement initiatives.
Keep current with Medi‑Cal, CalAIM, and other regulations to ensure compliance.
Participate in ongoing professional development, including trainings on cultural competence, motivational interviewing, and crisis intervention.
Collaborate with the team, remaining flexible to take on additional tasks and share best practices.
Requirements
Willingness to travel to Shasta County.
3–5 years of experience in case management, social services, or healthcare.
Familiarity with Medi‑Cal, CalAIM, and Enhanced Care Management practices.
Understanding of healthcare systems and local community resources.
Strong communication, empathy, and cultural competence skills.
Proven organizational and time‑management abilities.
Competence with case management software and related tools.
Completion of a pre‑screen assessment and a valid California driver’s license (Class C minimum) with a personal vehicle and liability insurance.
