Enhanced Care Management (ECM) Lead Care Manager - Lassen County Pacific Health Group’s ECM programs focus on addressing social determinants of health and providing community‑based services. As a Lead Case Manager, you will create care plans, guide members, arrange all necessary services, and build authentic, trusting relationships.

Build strong personal connections through frequent in‑person visits, becoming a pivotal support system.

Responsibilities

Conduct frequent in‑person visits to members’ homes, shelters, or community centers.

Assess member needs, gather insights, and address concerns on the spot.

Arrange all services required—medical appointments, transportation, housing, financial support, durable medical equipment, home health visits, meal delivery, etc.

Advocate for members’ rights, resolve insurance or administrative roadblocks.

Coordinate care conferences among healthcare teams and community organizations.

Maintain detailed records of assessments, care plans, and progress notes.

Use data and observations to refine care strategies and influence program evolution.

Stay current with Medi‑Cal, CalAIM, and other regulations to ensure compliance.

Continuously improve service delivery through feedback and collaboration.

Attend trainings, workshops, and webinars to sharpen skills in cultural competence, motivational interviewing, crisis intervention, and trauma‑informed care.

Qualifications

3–5 years in case management, social services, or healthcare.

Familiarity with Medi‑Cal, CalAIM, and Enhanced Care Management.

Understanding of healthcare systems and local community resources.

Strong communication, empathy, cultural competence, and teamwork.

Proven time management and attention to detail.

Competence using case management software.

Valid California driver’s license (Class C) and personally operable vehicle with liability insurance.

Successful completion of a pre‑screen assessment and MVR background check.

Pay & Hours Base pay: $25.00 – $29.00 per hour.

Full‑time (40 hours per week).

Shift: 8‑hour days, Monday to Friday, 8:30 am – 5:00 pm PST.

Work Location Hybrid remote operating throughout Lassen County—travel required to members’ locations.
