Base pay range $27.00/yr - $30.00/yr

As a Lead Case Manager, you won't just create care plans—you'll personally guide members at every step, arranging all the services they need to thrive and building authentic, trusting relationships along the way.

Responsibilities

Coordinate clinical visits and respond to real‑life challenges such as housing, food insecurity, and mental health.

Arrange all necessary services—medical appointments, transportation, safe housing, financial support—while ensuring no detail is overlooked.

Conduct frequent in‑person visits (multiple on‑site visits each month in members' homes, shelters, or community centers) to establish trust and gather first‑hand insights.

Use visits to provide tangible solutions (e.g., arrange durable medical equipment, home health visits, meal delivery).

Perform empathetic assessments, look beyond forms, and provide continuous support via phone, video, and in‑person visits.

Identify, coordinate, and optimize local resources such as housing assistance, job training, or childcare services.

Champion members' rights by pushing for timely treatments, insurance authorizations, and fair access while managing appeals processes.

Serve as the central point of contact to keep members, families, healthcare teams, and community organizations aligned.

Maintain meticulous records of assessments, care plans, and progress notes.

Use data and observations to refine care strategies and advocate for new partnerships or program changes.

Stay current with Medi‑Cal, CalAIM, and other regulations, ensuring all practices meet legal and quality‑of‑care standards.

Participate in ongoing learning—trainings, workshops, webinars—in areas such as cultural competence, motivational interviewing, and crisis intervention.

Support the team by taking on additional tasks and sharing best practices.

Qualifications

Willingness to travel to Santa Cruz County.

3–5 years of experience in case management, social services, or healthcare.

Familiarity with Medi‑Cal, CalAIM, and Enhanced Care Management.

Understanding of healthcare systems and local community resources.

Strong communication, empathy, and cultural competence.

Proven time‑management skills and attention to detail.

Competence using case management software and related tools.

Successful completion of a pre‑screen assessment.

Valid California driver’s license (Class C or higher), own vehicle, liability insurance, and clearance of an MVR background check.
