Key Responsibilities
Perform daily documentation of resident care, status, and changes in condition accurately and completely in compliance with facility policy and regulations.
Transcribe physicians’ orders to Medication Administration Records (MARs) and treatment sheets, ensuring meticulous record‑keeping.
Notify the facility pharmacy promptly of new or changed orders.
Prepare and administer medications and perform treatments as ordered, strictly within the LPN scope of practice in Wisconsin.
Monitor residents’ physical and emotional well‑being throughout your shift, reporting significant changes to the Registered Nurse or Director of Nursing immediately.
Supervise Certified Nursing Assistants (CNAs) on your unit to ensure delivery of high‑quality personal care.
Participate actively in the interdisciplinary care team to develop and implement individualized care plans.
Perform all other duties as assigned by the Unit Manager or Director of Nursing.
