Sign‑on Bonus:
$5,000k
Salary range: $80,000-$83,000
Job Responsibilities
Coordinate treatment and implement services for assigned clients, including outreach visits, as part of a multi‑disciplinary Early Childhood Services team.
Provide outreach services to children and families.
Consult with team members to develop individualized plans and multidisciplinary services for assigned clients.
Maintain client records accurately, including IFSPs, assessments, progress documentation, and required contact records with collaterals and family members.
Document assessment results, DPH forms, and regulatory compliance for intake and discharge.
Complete billing documentation according to schedule established by the company.
Adhere to professional standards and guidelines of the respective discipline.
Keep supervisor informed of client progress.
Meet direct service requirements based on part‑time or full‑time status.
Follow clinical recommendations from clinical administration.
Maintain confidentiality in all clinical matters.
Remain compliant with billing and documentation in accordance with direct service requirements.
Conduct client visits and relevant collateral visits in natural environments as needed.
Qualifications
Current licensure as an Occupational Therapist by the Massachusetts Board of Registration of Allied Health Professionals.
