Location: North Broward- Parkland, Coral Springs, Coconut Creek, Margate
Job Type: Independent Contractor (1099)
Pay: $50-$55 per visit

Position Summary We are seeking a Licensed Clinical Social Worker (LCSW) to provide in‑home services for Medicare Part B patients. In this role, you will assess psychosocial needs, develop care plans, provide counseling and resource coordination, and support patients and their families in managing emotional, social, and environmental challenges.
Key Responsibilities
Assess patients’ psychosocial needs and develop individualized care plans.
Provide counseling and emotional support to patients and families in the home setting.
Assist patients in accessing community resources, support services, and benefits.
Collaborate with the home health/therapy care team to coordinate care.
Document assessments, interventions, and progress in compliance with Medicare Part B regulations.
Educate patients and families on coping strategies, mental health management, and available resources.
Advocate for patient rights and ensure care plans align with patient goals and needs.
Adjust interventions as needed based on patient progress and changing circumstances.
Qualifications
Active LCSW license in the state of practice.
Master’s degree in Social Work from an accredited program.
Experience in home health, medical social work, or geriatric care preferred.
Knowledge of Medicare Part B documentation and regulations.
Strong assessment, counseling, and crisis intervention skills.
Excellent communication, organization, and problem‑solving abilities.
Reliable transportation and a valid driver’s license.
Comfortable working independently and managing a flexible, home‑based schedule.
CPR and other relevant certifications as required.
