Senior Case Manager / Case Manager – St. Hilda’s House
Position:
Senior Case Manager/Case Manager
Location:
Manhattan (Hamilton Heights)
Reports to:
Program Director, St. Hilda's House
Function:
Provides case coordination and delivers community rehabilitation services.
Schedule
Monday – Friday: 11:00 am – 7:00 pm
Sunday: 9:00 am – 5:00 pm
Monday: 7:00 am – 3:00 pm
Tuesday – Thursday: 9:00 am – 5:00 pm
Responsibilities
Conduct regular home visits for assigned caseload.
Obtain psychosocial histories and perform functional skills assessments.
Establish and maintain liaison with primary psychiatric providers.
Ensure access to psychiatric, substance abuse, and medical services, and entitlement benefits.
Administer standardized health screening tools.
Monitor client medication, including adherence and side‑effects.
Develop individualized rehabilitation plans (Service/Support Plans) in consultation with the treatment team.
Deliver community rehabilitation services such as assertiveness training, community integration, daily living skills, health services, medication management, parenting training, counseling, skill development, socialization, substance abuse services, and symptom management.
Document client responses, progress and regressions, and update plans regularly.
Train clients in safety procedures and conduct fire drills and self‑preservation tests.
Prepare residents for transition to independent living and make necessary referrals.
Facilitate crisis intervention during emergencies.
Staff the emergency phone on a rotating basis.
Maintain client charts and submit required notes and reports, including incident reports.
Oversee apartment maintenance, inventory furniture, and alert the program director to maintenance issues.
Attend in‑service training as requested.
Provide coverage as needed for other staff on the program team.
Serve as a “Qualified Mental Health Staff Person.”
Perform duties assigned by the supervisor.
Qualifications
Senior Case Manager:
M.A. in psychology or a human services related field, or B.A. with a minimum of 3 years of direct care experience. Master’s preferred. Experience with adults with serious mental illness is preferred.
Salary: $58,034 plus generous benefits.
Case Manager:
B.A. or A.A. in psychology or a human services related field, with a minimum of 3 years of direct care experience. Experience with adults with serious mental illness is preferred.
Salary: $50,078 plus generous benefits.
Strong written and verbal communication skills required.
