Location & Schedule Address: PWC 337 E. Virginia Ave, Phoenix, AZ 85004 Schedule: Monday - Friday 8:00 AM - 5:00 PM

Position Summary Under the direct supervision of the Clinical Director, the Behavioral Health Assessor will complete all client assessments in a timely manner and recommend clinically appropriate care based on assessment outcomes. Placement may be within NAC programs or with other agencies providing appropriate levels of care.

Responsibilities

Complete all assessments and related documentation.

Recommend appropriate services based on assessment outcomes and make referrals to internal or external providers.

Update assessments and ASAM as needed.

Develop Individual Service Plans in collaboration with clients.

Assist with case management as required.

Work closely with the admissions team to prioritize residential admissions for Native American, pregnant, IV substance use, homeless, etc.

Notify clients and referral sources of all documents and requirements for entering treatment prior to arrival.

Ensure all documentation is entered into the electronic health record within 24 hours.

Maintain positive relationships with T/RBHAs, Phoenix Area Indian Health Service offices, tribes, detox centers, Phoenix Indian Medical Center, and other referral agencies.

Assist in conducting group or individual counseling as outlined in the clinical curriculum.

Support and engage in cultural inclusion practices and CLAS standards.

Attend regular clinical supervision and oversight meetings.

Perform other duties as assigned.

Educational / Work Experience Requirements

Tier I ($55,000 – $64,000)

– Master’s degree in a behavioral health field required; 2 years’ experience in behavioral health or substance abuse preferred.

Tier II ($65,500 – $71,500)

– Associate-level licensure in a behavioral health field (LMSW, LAC, LAMFT, or LAAC); 2 years’ experience preferred.

Tier III ($70,000 – $91,000)

– Independent licensure (LCSW, LPC, LMFT, or LIAC); 2 years’ experience preferred.

Skills Requirement

Fingerprint certificate within 90 days of hire.

MS Office proficiency.

Strong interpersonal skills with teams, managers, supervisors, and vendors.

Excellent written and oral communication skills.

Critical thinking, problem-solving, organizational, and time management skills.

Bilingual – Spanish desired.

Required strong clinical skills.

Knowledgeable about the economic, educational, and social challenges of Native Americans.

Familiar with assessment tools for substance abuse, depression, and other behavioral health issues.

Knowledgeable about the DSM (latest revision) and diagnostic criteria.
