Responsibilities
Assist the Physical Therapist in carrying out treatment plan and gather objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change screenings.
Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.
Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.
Maintain accurate and timely documentation of resident progress and interventions. Comfortable and knowledgeable with Point of Service (PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conduct remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other rehab staff.
