Responsibilities
Assist the Physical Therapist in carrying out treatment plans and gather objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change screenings.
Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.
Observe residents' performance, report changes or concerns to the supervising Physical Therapist.
Maintain accurate and timely documentation of resident progress and interventions: PoS care delivery, detailed evaluations, treatment plans, daily and progress notes, HMO/Managed care progress updates in EMRs (Net Health/PCC).
Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress; conduct remote check‑in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
Collaborate with the interdisciplinary team following guidance of the Physical Therapist and in cooperation with other rehab staff.
