Active, unencumbered LPN/LVN license in the state of practice
Compact licenses transferred within 90 days of moving
Current CPR certification (hands‑on component); company can assist after hire if needed
TB skin test current within last 12 months
Six months of hands‑on nursing experience preferred but not required
Reliable transportation
