Job Title NUTRITIONIST II WIC – PHN II

Overview The primary purpose of this position is to provide nutrition services and breastfeeding support for the Bladen County WIC program. Responsibilities include conducting assessments, reviewing client data, monitoring ongoing condition, providing education, completing food prescriptions, referrals to health‑care and public‑assistance programs, and coordinating breastfeeding services. The role also entails developing care plans, evaluating outcomes, recording communications, and collaborating with medical providers.

Responsibilities

Provides direct nutritional services to patients with complex nutritional needs, including women with diabetes, hypertension, obesity, anemia, gestational diabetes, infants with low birth weight or medical complications, and children with developmental or chronic conditions.

Receives and evaluates referrals for WIC patients from various health providers and non‑WIC patients as time allows.

Completes nutrition assessments and examines food preferences.

Conducts interviews to obtain health and professional information and extracts data from medical records.

Develops patient‑specific care plans related to the patient’s needs and condition.

Consults on therapeutic diets, nutrition principles, and dietary habit modifications.

Provides breastfeeding promotion, education, and support.

Communicates with local agencies, physicians, and medical centers to ensure all patient needs are met and makes necessary referrals.

Plans and implements the WIC Nutrition Education program with other ongoing nutrition education initiatives.

Evaluates the nutritional status of WIC applicants to determine eligibility through anthropometric and biochemical data assessment.

Prescribes appropriate food packages to WIC participants tailored to individual needs.

Documents all entries in the patient’s medical record.

Breastfeeding Promotion and Support

Provides breastfeeding education, counseling, and support through classes, supplies, referral systems, and coordination with the breastfeeding peer‑counselor program.

Establishes and maintains collaborative partnerships for breastfeeding promotion with community health providers, businesses, faith organizations, schools, and child‑care facilities.

Ensures a clinic environment that supports breastfeeding.

Completes breastfeeding assessments and develops interventions for breastfeeding mothers and infants at initial, subsequent, and mid‑certification nutrition education appointments.

Knowledge, Skills, and Abilities

Considerable knowledge and ability to apply nursing theory, practices, principles, and techniques in public health settings.

Strong understanding of principles and practices of public health and available resources.

Effective partnership skills with patients and other service providers to elicit needed information and maintain relationships.

Computer proficiency for email communication, patient record documentation, and information retrieval.

Education and Experience Requirements Graduation from a four‑year college or university with a B.S. in Nursing that includes a Public Health Nursing rotation and one year of Public Health Nursing experience; or Master’s in Public Health and a nursing degree with one year of professional nursing experience; or a nursing degree with two years of nursing experience including one year of Public Health Nursing experience.

License or Certification Required

Licensed to practice as a Registered Nurse in North Carolina.

Valid North Carolina Driver’s License.

CPR license.

Special Requirements

Valid North Carolina Driver’s License.

State Breastfeeding Training Program.

May be required to obtain additional certification(s) as deemed necessary by supervisor.

For a nurse without a B.S. degree in Nursing, must complete Introduction to Public Health Nursing.

Must successfully complete the North Carolina Lactation Educator Training Program within one calendar year of assuming the position and complete training every ten years as part of ongoing training.

Must attend 20 hours of continuing education in breastfeeding every five years approved by the International Board of Lactation Consultant Examiners.
