Job Description
As a valued member of the Bluebonnet Trails Community Services Youth and Family services team, this professional position conducts a comprehensive assessment of the youth and their families, working in full partnership with team members to develop a plan of care, overseeing implementation of the plan, identifying providers of services or family based resources, facilitating child and family team meetings, and monitoring all services authorized for a child’s care.
The Wraparound Facilitator assures that care is delivered in a manner consistent with strength-based, family driven, youth guided, and culturally competent values, offers consultation and education to all providers regarding the values and principles of the model, monitors progress toward treatment goals, and ensures that necessary data for evaluation is gathered and recorded.
The person in this position requires an aptitude for working with people who have a mental health diagnoses as well as evidence-based practices in the promotion of recovery for individuals with severe and persistent mental illnesses. This position allows opportunities for skilled professionals to use their knowledge and skills in assessment, collaborative person-centered recovery, differential diagnosis as well as wellness and recovery-oriented systems of care.
Locations Fayette and Lee Counties
Hourly Rate $22.22 – $27.97/hour
Salary Differentials
$0.75/hr Bilingual
$1.00/hr Human Services Master’s degree
$1.00/hr 1 year with BTCS
$1.00/hr 2 years at BTCS
$1.00/hr 4 years at BTCS
$1.00/hr 6 years at BTCS
$1.00/hour – Advanced Licensure
Service Delivery Incentive $200/month

Minimum Qualifications
A Bachelor’s degree from an accredited college or university in a social, behavioral, or human service field or documented coursework equivalent to a major in human services.
Knowledge of the effects of mental illness.
Must have a valid Texas Driver’s License and maintain approved driver status throughout employment.
Preferred Qualifications
One year of experience in mental health assessment and/or crisis intervention and screening.
Master’s degree in psychology, social work or a related human service field.
Bilingual preferred.
Salary $46,217 - $58,177/year
