Overview Competitive compensation and flexible scheduling. Sign‑on bonus:

Responsibilities During a visit that can last up to one hour, providers review and observe a member’s health, medical history, medication adherence, social environment and other risks to gain insights that are hard to capture during routine office visits. Our Comprehensive Health Assessment (CHA) helps improve quality of care and close multiple care gaps with a single visit.
Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case‑management referrals, follow‑up and clear documentation according to Matrix guidelines and protocols.
Administer and order point‑of‑care and lab‑based testing as needed.
Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
Collaborate with the Primary Care Physician on patient education and follow‑up as necessary.
Provide services at the member’s place of residence, which can be in their homes or Skilled Nursing Facilities.
Benefits of Working at Matrix
Flexibility – visits are scheduled based on your availability. Assessments run Sunday–Saturday from 8 am–7 pm. You can work during the day, evening, or weekend.
Competitive pay – earn income based on the number of assessments completed. Those completing a monthly average of 20–100 visits earn $1,800–$11,500 per month.
Benefits – mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.
Education and support – comprehensive onboarding, a preceptor, 1:1 feedback, and free access to continuing education.
Additional licensure – support in obtaining additional state licensure and credentialing in neighboring states or others you may wish to visit.
Full‑time opportunities – PRN providers can transition to FT roles with a sign‑on bonus.
Qualifications
Master’s degree or equivalent experience and satisfactory completion of NP licensure.
Current NP licensure in state of practice including prescription authority or ability to obtain it as needed.
AANP, AACN or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency or Acute Care specialty.
Current BLS, ACLS or CPR certification.
1 year of NP experience preferred; new graduates encouraged to apply.
Valid state driver’s license and proof of adequate automobile insurance coverage for state of residence.
Strong computer skills and familiarity with employee health/medical record software.
Excellent verbal and written communication skills.
