Overview
Matrix Medical Network pioneers the first national in‑home clinical network. We offer competitive compensation, a $2,000 sign‑on bonus, and flexible scheduling. Join our virtual hiring events on January 14 at 5 pm EST or January 15 at 9 am EST to learn about our in‑home health assessments.
Responsibilities During an up‑to‑one‑hour visit, a provider reviews a member’s health, medical history, medication adherence, social environment, and risk factors. The Comprehensive Health Assessment (CHA) targets lost time in routine office visits.
Conduct adult/geriatric assessments with documentation per Matrix guidelines.
Administer and order point‑of‑care or lab tests as needed.
Collaborate with physicians, case managers, social workers, family members, and other medical personnel.
Coordinate with a primary care physician on patient education and follow‑up.
Provide services at the member’s residence, including homes and skilled nursing facilities.

Competitive Pay
– Earn $90‑$115 per assessment; average 20‑100 visits yields $1,800‑$11,500 monthly.

Additional Licensure

Qualifications
Master’s degree or equivalent experience with satisfactory NP licensure.
Current NP licensure in state of practice, with prescription authority or the ability to obtain it.
AANP, AACN, or ANCC board certification in relevant specialties (Adult, Adult Gerontology Primary Care, etc.).
Current BLS, ACLS, or CPR certification.
1 year NP experience preferred; new graduates encouraged.
Valid state driver’s license and proof of automobile insurance.
Strong computer skills and familiarity with medical record software.
Excellent verbal and written communication skills.
