Matrix Medical Network pioneers the first national in-home clinical network and now provides comprehensive health assessments nationwide. We deliver personalized care that improves health, lowers costs, and empowers people to live their best lives. Our clinicians meet at members' homes or facilities to identify chronic conditions and close care gaps with a single visit.

Responsibilities
Conduct adult/geriatric assessments that include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and documentation according to Matrix guidelines and protocols.
Administer and order point-of-care and lab-based testing as needed.
Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
Coordinate with Primary Care Physicians on patient education and follow-up.
Provide services at the member’s place of residence, including homes or Skilled Nursing Facilities.
Benefits Of Working At Matrix
Flexibility – Visits are scheduled based on your availability. We conduct assessments Sunday–Saturday from 8 am–7 pm. You can work during the day, evening, and/or weekend.
Competitive Pay – Earn income based on the number of assessments you complete. Those completing 20–100 visits per month earn $1,800–$11,500, depending on state and per‑assessment rate ($90–$115).
Benefits – Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.
Education and Support – Comprehensive onboarding, a preceptor, 1:1 feedback, and free continuing education.
Additional Licensure – Support for obtaining additional state licensure and credentialing as needed.
Full-time Opportunities – PRN providers can transition to full‑time roles and receive a sign‑on bonus.
Qualifications
Master’s Degree or equivalent experience and satisfactory completion of NP licensure.
Current NP licensure in state of practice with prescription authority or ability to obtain it.
AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
Current BLS, ACLS, or CPR certification.
1 year of NP experience preferred; new graduates encouraged.
Valid state driver’s license and adequate auto insurance coverage.
Strong computer skills and familiarity with employee health/medical record software.
Excellent verbal and written communication skills.
