Required for after hours and weekend coverage on a rotating basis.

Position Purpose:

The primary purpose of this position is to provide primary medical management in collaboration with the PACE Organization (PO) Physician(s). Performs assessments in collaboration with the physician as needed, develops and implements appropriate plans of care to PO participants, evaluates participant physical complaints and provides appropriate treatment, offers participant and caregiver teaching and education, and functions as a member of the Interdisciplinary Team (IDT). Regularly informs the IDT of each participant’s medical condition, remaining alert to pertinent input from other team members, and documents changes in the participant’s medical record consistent with documentation policies.

What You Will Do:

Provide primary medical coverage in collaboration with PO physicians as needed. Perform initial, semi‑annual, and annual assessments, and additional assessments as required, implementing appropriate plans of care.

Participate in discharge planning for acute and long‑term placement.

Evaluate and treat participants during acute illness. Prudently prescribe medications, therapies, and other treatments for participants. Refer participants to medical specialists as indicated following consultation with the Primary Care Physician.

Make nursing home or home visits when needed.

Work with the Medical Director and Clinic Supervisor to formulate clinical policies, procedures, and standards of care. Assist with the development of standards of care; perform ongoing monitoring and evaluation of patient care practice and service delivery; provide guidance and training to staff regarding medical and quality assurance issues.

Advise the Medical Director and Primary Care Provider to establish better accountability of the PO services to participants and referral sources.

Act as a resource during intake of new participants and in the day‑to‑day operation of the center programs.

Participate in provider on‑call rotation as required.

Minimum Qualifications:

Master's degree in Nursing.

Five (5) to seven (7) years of experience, preferably in long‑term care.

Current, unencumbered, active license to practice in the state of PA.

CDS Certification and DEA Registration.

Board Certification by ANCC or AANP.

Certification as a Nurse Practitioner with prescriptive authority.

Current BLS certification.

Minimum of one (1) year working with the frail or elderly population strongly preferred.

Ability to act decisively, focus on customers, maintain high decision quality, adapt to ongoing change, and exhibit organizational agility.

Valid driver’s license, proof of insurance, and means of transportation.

Proficiency with the Microsoft product suite (Word, Excel, PowerPoint, etc.) and basic knowledge of electronic mail and calendaring systems.

Periodic travel required within the Centers geographic region.

Ability to perform frequent kneeling, crouching, reaching, lifting, sitting, walking, bending, and lift a minimum of 20 pounds when applicable.

Programs of All‑Inclusive Care for the Elderly (PACE) or LIFE programs coordinate and provide all needed preventive, primary, acute and long‑term care services so that older individuals can continue living in the community. Many PACE participants receive most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.
