Position Purpose Join us in providing primary medical management in collaboration with our esteemed PACE Organization (PO) Physicians. As a Nurse Practitioner, you’ll conduct assessments, develop care plans, and ensure participants receive comprehensive care while fostering a warm and supportive environment.
What You Will Do
Collaborate with PO physicians, performing assessments and implementing care plans
Participate in discharge planning and provide acute illness treatment
Conduct home visits when necessary
Assist in formulating clinical policies and procedures
Participate in provider on-call rotation
Minimum Qualifications
Master’s degree in Nursing with five to seven years of experience, preferably in long-term care
Must possess a current, unencumbered, active license to practice in the state of PO
CDS Certification and DEA Registration required
Board Certification by ANCC or AANP
Certification as a Nurse Practitioner with prescriptive authority required
Current BLS required
A minimum of one year working with the frail or elderly population or, if less than one year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring
Ability to travel within the Center’s geographic region
