About the Program and the Family Nurse Practitioner position: Our Transitional Foster Care Program for Unaccompanied Children (UC) overseen by the Office of Refugee Resettlement (ORR), ensures the well-being and safety of unaccompanied children (ages 0-17) who cross the US Southern border without an adult. Unlike traditional congregate care settings, our approach involves placing unaccompanied children in foster homes while diligently searching for and assessing potential sponsors identified in the US. During their stay with us, we provide medical care, case management, engaging life skills classes, and essential mental health services.

Our Family Nurse Practitioners play a crucial role in assessing and ensuring the health and well-being of our accompanied children who are trying to establish a new home in the United States. Our Family Nurse Practitioners support and treat any medical needs required by the children in our program. They work collaboratively with other team members involved in the child's care to provide appropriate services .

How you get to use your skills as a Family Nurse Practitioner…

Conduct initial medical assessments for each youth who are arriving from the border or being transferred from another program

Diagnose and treat basic medical conditions identified at time of initial assessment or during a sick-call

Direct care responsibilities include, but are not limited to: administering immunizations, managing wounds and infections, writing prescriptions, interpreting ECGs, drawing blood, etc.

Be able to place and interpret PPD skin tests for youth and for staff

Conduct follow-up services and refer youth to appropriate medical specialties when necessary

Create and electronically submit Treatment Authorization Requests and other required medical paperwork

Create and update information in electronic medical record systems (Unaccompanied Children Portal and MEDENT)

Discuss medical conditions with the Office of Refugee Resettlement and the Federal Field Specialists; ensure all regulations are met

Work collaboratively with all members of the medical team, the Case Manager, Clinician, and Skills Trainer to provide appropriate service to all youth in our programs

Required Experience of a Family Nurse Practitioner:

Appropriate NYS certifications and licensures - required

Experience working with at-risk youth - strong preferredStrong communication and organizational skills - required

Experience with Microsoft Office (Word, Excel, etc) and electronic medical records systems (MEDENT)

Ability to handle confidential material in a responsible manner

Ability to work effectively in a team context

Demonstrated ability to perform duties with minimal supervision and to exercise appropriate judgement

Ability to travel to other agency locations

Physical Requirements of a Family Nurse Practitioner:

Word processing: Including responding to emails, composing letters, completing computerized documentation, etc

This involves sitting at a computer for several hours

Completing paperwork such as copying, scanning, and faxing documents

Must be able to lift up to 50lbs

Schedule of a Family Nurse Practitioner: Exempt

Must be flexible to meet program needs

On-call and holidays may be required
