Job Description Work Schedule:
80% FTE, Monday – Friday between the hours of 8:00 AM – 5:00 PM. No holidays or call. Hours may vary based on the needs of the department.

As an Advanced Practice Provider, you will play an integral role in building and expanding a virtual primary care program. You’ll be a part of the ambulatory care teams by providing high-quality, cost-effective care to patients both in person and virtually.
We are seeking a
Nurse Practitioner OR Physician Assistant
to:
Be a part of a virtual care team expanding access to primary care through telehealth visits
Care for the acute and chronic needs of patients from newborn to elderly.
Perform physical exams and manage the health maintenance needs of our family medicine population.
Perform routine clinical procedures independently including wart removal, PAP smears and skin lesion removals.
Education:
If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.
If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program.
Work Experience:
Required – Two (2) years of relevant clinical experience
Licenses and Certifications:
Minimum – State of Wisconsin NP or PA licensure
Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice
Active board certification as a NP in area of clinical practice (e.g., Family)
Active PA license, including prescriptive authority, in state(s) of practice
Active PA board certification
Active DEA license or ability to apply for such license prior to or upon hire
All Nurse Practitioner and Physician Assistant applicants must have active DEA license or the ability to apply for such license prior to or upon hire.
Qualifications
Master's or Doctorate’s degree from an accredited Nursing Program
Two years of relevant clinical experience
State of Wisconsin NP or PA licensure with prescriptive authority
Active board certification as a NP in a clinical practice area (e.g., family, gerontology, etc.)
Active PA board certification
Active DEA license or ability to obtain one prior to or upon hire
