Schedule / Shift :
Full-time : Monday-Friday, 8:30 AM - 5:00 PM
Part-time : Wednesday and Thursday, 8:30 AM - 5:00 PM (potential for weekend Urgent Care hours)
Job Description Responsibilities / Job Duties :
Triage patients by telephone or in person to assess urgency and determine the appropriate level of care.
Perform patient procedures, assess health status, and provide health education regarding clinical conditions, treatments, follow-up actions, and home care.
Communicate abnormal test results to providers.
Track patients for necessary follow-up and document appropriately.
Collaborate with MAs, providers, and the SUD team to address patient concerns and ensure comprehensive care.
Maintain accurate patient records and support overall team efforts to deliver patient-centered care.
Minimum Education and Experience Qualification Requirements :
Massachusetts RN License
BLS Certification
Experience working with diverse patient populations preferred
EPIC experience, bilingual skills, and FQHC experience are a plus
